MAKE A DIFFERENCE! Transitional Living & Community Support (TLCS) is a non-profit social rehab agency providing services to psychiatrically-disabled adults who are homeless/on the verge of homelessness.

DAYS/HOURS: Non-exempt; Full-Time; Temporary Position (less than 6 months duration)

DESCRIPTION: The Substance Abuse Specialist/Case Manager coordinates all services and resources to clients on their caseload. The goal of the Substance Abuse Specialist/Case Manager is to provide referrals, crisis intervention, support and encouragement to enable the client to choose, find and keep permanent housing. This position assists clients with vocational/ employment issues, as well as financial matters and entitlement issues as applicable. The Substance Abuse Specialist/Case Manager will be responsible for providing direct services including substance abuse assessments, individual and group counseling, and education and information to benefit clients who are dually diagnosed. This position will work closely with other program staff and community substance abuse resources.

MINIMUM QUALIFICATIONS:
At least 2 years experience working with psychiatric disabled individuals.
At least 2 years work experience with persons who have substance abuse problems, preferably with people who are dually diagnosed (mental health & substance abuse). Personal experience with recovery will be considered.
Previous work experience with MediCal billing.
Demonstrated mastery of basic writing skills, including the ability to write/maintain clear and accurate case notes and reports.
Excellent oral communication skills.
Ability to interview, assess for substance abuse and make an appropriate intervention plan for adults with a psychiatric disability and with substance abuse problems.
Ability to advocate for and coordinate community services and housing options for dually diagnosed clients.
Ability to work within multiple models, including but not limited to 12-step, Harm Reduction, faith based and non-faith based.
Ability to communicate and interact with adults who have a psychiatric disability.
Ability to communicate and interact with adults who have acute and/or chronic physical health issues.
Ability to assess emergency situations and provide crisis intervention.
Ability to teach independent living skills.
Ability to work cooperatively with other staff.
Ability to work under pressure and in stressful situations.
Ability to manage multiple priorities.
Must be computer literate with experience in word processing and email familiarity; Microsoft Office preferred.


THE SUCCESSFUL CANDIDATE MUST ALSO HAVE:
A car
Valid CA driver’s license & vehicle registration
Current auto insurance in accordance with California State Law
Must have at least 3 years of driving experience
Driving record must meet, and continue to meet, the underwriting standards established by the generalized commercial driving guidelines used by our insurance broker.
Must be able to pass clearance of a Department of Justice and/or Federal Bureau of Investigation background screening.

POSITION OBJECTIVE

The Personal Service Coordinator II/ Substance Abuse Specialist provides direct client community support and case management, education to clients. Additionally, Personal Service Coordinator II/ Substance Abuse Specialist is responsible for providing members with the provision and linkage to substance abuse treatment resources. Responsibilities also include helping all members who request substance abuse treatment, assessment/support to those in various stages of recovery, collaborates with agencies such as the Conservator's Office, Public Defender's Office, Child Protective Services, Probation Department, California Department of Corrections and Rehabilitation, County Mental Health Services and other health service providers. Flexible work schedules may be required, including evening and weekend hours and 80% of services are provided in the field, requiring the use of a personal vehicle. This position requires 24/7 coverage (including crisis phone line and on-site response) as needed.


QUALIFICATIONS

-Bachelors or Master’s degree in a related field is required, as well as one year experience providing substance abuse services in the community. Coursework and/or certification in Drug and Alcohol counseling are preferred.

-Additional experience working with persons experiencing serious mental illness in an inpatient or outpatient setting a plus

-Must have valid California driver's license, access to a car for the use of community visits/transporting clients and pass a motor vehicle safety inspection

-Must possess proof of insurance and receive a motor vehicle record clearance

-Ability to work effectively as a member of a culturally competent multidisciplinary team, a thorough understanding of member rights, an understanding of the conservatorship process, knowledge of basic medical terminology, psychiatric diagnoses, clinical interventions service planning and documentation of services

-Strong communication skills (written/oral); fluency with additional languages is a plus

-Document services in a comprehensive manner and basic computer skills are required

-Ability to handle unpredictable schedule, adapt and respond to crises are required

-Applicant must be fingerprinted and receive clearance from the Department of Justice and the Office of Inspector General
